State PKR communications director Amir Ghazali said the 12 have signed a declaration to back Anwar amidst "challenges" to his leadership.
"Only Nibong Tebal chief Mansor Othman haven't not signed the document but he has agreed to support Anwar. He is busy in Parliament today," Amir told Malaysiakini.
He said the 12 held a meeting today and promised to stand by Anwar through "thick and thin".
When contacted by Malaysiakini, Mansor replied with a terse "I will not sign" without elaborating.
However, a source close to Mansor said he did not sign the document after losing faith in the party leadership.
"Whether he supports or not, it means nothing any more. The party is chaotic and has lost its purpose. That's why he didn't sign," he said.
Penang is the third state after Perak where the vast majority of division heads pledged their allegiance to Anwar.
In the case of Perak, 22 out of 24 division chiefs made the pledge, with only Abdul Yunus Jamhari (Parit Buntar) and Chua Yee Ling (Bukit Gantang) abstaining.
On July 20, 16 out of 23 Johor PKR division heads made a similar pledge.
PKR division heads are elected, except in cases where there are pro-tem committees.
Tit-for-tat
Both pledges by Penang and Perak PKR division heads were similar in content to a pledge signed by 22 national party leaders at the party's leadership retreat over the weekend.
There have been several shows of numbers since last week by Anwar and PKR deputy president Mohammed Azmin Ali's supporters.
Azmin's supporters got the ball rolling on July 18 when a joint statement was issued by a group of PKR MPs and elected central leadership council (MPP) members.
This group had urged Anwar to stop making remarks that could divide the party, in reference to his suggestion that Azmin should step down should allegations of sexual misconduct be proven true.
