A Malaysian-born neurosurgeon accused of killing two escorts has been refused bail.

Justice Megan Latham concluded that the extent of Suresh Surendranath Nair's cocaine addiction meant it was likely he would commit more serious offences if he was granted bail, the Australian Associated Press reports.

The 41-year-old is charged with causing the death of Victoria McIntyre, 23, in circumstances amounting to manslaughter - by an unlawful and dangerous act - on Feb 15 last year at his Elizabeth Bay flat in Sydney's east.

He also is charged with murdering Suellen Domingues Zaupa, 22, on November 19 last year at the same address.

In the New South Wales Supreme Court Tuesday, Justice Latham said she could not characterise the crown case for the murder charge as "strong".

She noted that before Nair was charged over the deaths, he had been granted bail for offences relating to supplying cocaine to one of the women.

But he had been taken back into custody after breaching those bail conditions when he was found in the company of two naked escorts and in possession of cocaine.
